I would say you are getting a little excited there. I am not 100% sure but I would think that form would need to be filled out closer to the trip. At least 8n the same year as the trip. Maybe try a couple weeks before you go.

The form can be filled out after you have booked your flight according to this web site, https://www.jacustoms.gov.jm/article...house-weekly-6, or even on route to Jamaica. You can also fill out a paper form once you land in Jamaica.
They also have kiosks at the reception hall for use by visitors.
I would wait till closer to departure then try again.
We did the on-line form back in April 2022, but didn't see it as helping much. Seemed we still went through the same arrival process.
